September 8, 1912, at the commune office in Guardia Lombardi, the local official having received from Luigi Valvano a copy of a death act transcribed it into the local record. The transcription reads

Consular Agency of P.M. the King of Italy in Scranton, United States, with certified translation into Italian. Death Certificate, of Vito M. Valvano. Scranton, Pennsylvania, office of the state health department.

Doctor affirms having treated him from March 14 to March 15, 1910, and he died at 1 a.m.
Cause of death: Internal stomach injuries incurred in a mining accident.

He was buried in Catholic Cemetery of Blakley on March 15, 1910.

The rest is attestations of the funeral director, doctor, translator, consular officers, and local commune officials as to accuracy of the document and the insertion in the commune register.
