Update: Although I thought I followed all the instructions, my mailed, notarized application for my mother's birth record was declined because I needed to include her death record. This information was not included in any of the instructions I read. Just wanted to share that info with you and others ...

I have the same issue and I'm hoping the Consulate will understand the misspellings of the names, especially because of the language barrier.
I don't think it would hurt to have the other non-required documents, such as the other census records, to show the names of the family ...

Just be prepared to wait. I sent for my mother's and my birth record in early February and was told today that they haven't even gotten to those mailed requests yet! I was also told it could take up to another five-to-six weeks before I receive the records.
