Help requested on how to write a commune

With all of the help I’ve had on this site and some information from my grandma saying my G Grandpa Samuel Marion (Sebastiano Di Maria) birth date was on Sept. 18th but she told me 1898 but I don’t think she is right. Also this ww1 document is the same birthdate and town he was born in

I am requesting some help on how to write the town, what town/place (the address) I should send it too and what to say. To ask for his birth act. Since the documents aren’t online for the year 1894 (any reason why?) thank you for all of your help.

There are form letters at Family search for writing to Italy in Italian.

Thank you for the info! The problem is, I can’t read Italian 😂 but I would like to write to ask for his birth act. Does anybody know why documents from 1894 in that town aren’t online?

If you use the form below, and fill in the blanks in English, you can hit translate, and it will translate your letter into Italian. You don't need to be able to read or write Italian to do this, and you can choose the option for birth record. The spaces where you see downward arrows give you menus which allow you to select what info you want in those spaces. I don't know why the 1894 birth records are missing from the state archives, but the town itself may have the info you need, which is why you should contact the town directly.
