What words do Italians use for step- and half-relationships?

In the Italian language the suffix -astro has a derogatory meaning. Words as fratellastro or sorellastra seem 'bad' words. I don't think they were very used even in the past, but now they seem to me out of use. No one, I think, would introduce a person saying "Questo è il mio fratellastro", but simply "Questo è mio fratello". Not necessary to point out that they have a different father or mother. Should this be indicated, such as for legal reasons, the technical terms should be "fratelli germani" (same father and same mother) and "fratelli unilaterali" (only one common parent).

I was wondering about in documents more than in conversation.
