translating section of birth record

Hi everyone
Just needed help translating the bottom section of my relative's birth record.
I also noticed that the address he was born at was different to his siblings - not
sure if this is explained??
Any help would be great!

The present declaration was made by Cristiano (the midwife) who had assisted with the birth, and in place of the mother who was infirm, and of the Father who was outside of the town.
I read the present act to the participants who signed below with me, except the declarant who was not able.
