Would someone kindly translate the gist of this birth record? It is the record for Alessio Capua - order number 4
It appears he may have been a foundling.

On 5 September 1888, Maria Rosata Nestico, age 38, daughter of Rosario, presented a child to the official in the town, who she declared she found at the door of her home in rione Vaglio #2 in this town. The child appeared to be in good health, about 1 day old, with no identifying marks or items. The official gave the child the name Alessio Capoa. Maria Nestico asked that the child be consigned to her to be cared for, and the official did so.

Do you know why they would have assigned that particular surname?

Many towns assigned names like "Proietti" or "Trovato" or "Esposto" which clearly labels a child for life as an orphan/foundling. So officials began assigning "fantasy" names to these children. The name was made-up and usually was not a name found in the town.
This official came up with Alessio Capoa.
If this child was later "recognized" by its parent(s), there would be another birth entry in whatever year the recognition was made.
Do you know this child as an ancestor? Do you know if he married in Italy?

Yes, he did. He ended up marrying into the same Nestico family that adopted him. Curiously, he completely changed his name (first name and surname) when he naturalized as a US citizen.

I see in the publication of marriage banns (intent to marry) that his surname is spelled CAPUA.

It is interesting to note that due to Alessio's age (24) he was considered a minor. In both records it states that he is given consent to marry from the "Consiglio di tutelo" - Guardianship council.
You mention earlier that he was adopted. That is not the case. He may have been raised by and considered part of the family (Nestico), but not officially adopted.
