This looks like a birth record stating that Rosa was born in Washington County, New York.
Here is her death record that states she was born in AMERICA.

Her father is Salvatore Gugliotta and her mother is Carmeela Dorotea Lo Presti.
Does it give her date of birth? Also, what is all the writing on the right side of the page say?
Is there any other important information in the record?
I would like to pre-thank anyone that helps me. I am entering this at my local Library as my home IP address has been blacklisted on this site by Spamhaus. Two long emails from Spamhaus and 3 long phone conversations with my ISP provider have not solved this issue. I have emailed Italian Genealogy website twice with no response. I'm reading her birth date as 2 October 1915, in Salem, Washington county.
That would work with a death date of 4 October 1918 at age 3.
Right side notes the act of recognition as daughter of Salvatore Gugliotta, son of deceased Filippo, and of Dorotea Carmela Lopresti daughter of Giuseppe, with their marriage of 22 February 1910... then stated again with date of 22 February 1917. Probably just a correction of the year.
It is possible that the declarant, Giuseppe Lo Presti, age 57, was Rosa's grandfather, though a relationship is not given.
