How fussy is the Chicago consulate about discrepancies?

I am helping my cousin to apply and he will go through Chicago. His family name is Dorazio, sometimes spelled D'Orazio, and his grandfather was born Ignazio and later used Eugene (appears on early censuses as Gino when a child). Any thoughts about whether or not these differences will cause problems in Chicago? I am currently applying through NY and know they would be a problem here. Thanks!

From all that I've read, Chicago is not much better than New York in terms of difficulty.

For a first name americanizaton I used the Italian slef certification, which you will find on this site. The surname change may be another story, but it does appear close again I would use the self certify. For Chicago I had the standard Gusippie to Joseph and Antonio to Tony.
