Depends on the time period of the marriage Act. the usual way is a reference/note to the fact that in the attachments is the death act, or some certification of the death, of the first spouse.
The way to be sure is to get your hands on the allegati to the marriage.
But without context of a time period and a place your question is hard to answer.

TIP: When asking for records from Italy, do NOT ask for an "estratto." ALWAYS ask for a "copia integrale." A photocopy of the original Act will contain more information

In addition to what Anizio 
has stated above - (a reference to a death act)
After the name of the groom you will often see "celibe" (unmarried) or "vedovo" (widowed).
For a bride it would read "nubile" (unmarried) or "vedova" (widow).

Yes Tessa is right, that same thing also will tell you if someone dies without ever having married,
but the absence of those things mean nothing. I just checked some of my families second marriages and the words were missing, and the death of the first spouse was not noted on the marriage, yet in the allegati was a certifications of the husbands first wife's death.
So if "celibe" or "vedovo" or a reference to a first spouse exist, they are indicative. But if those things are not present it could be either. Allegati are the only sure way to know for sure absent those indicators.
