Before the official...

On the 5th of the month of June in the year 1952, at 5:20 PM, at the house at Via Gustavo Biancini #9, to Sara Milano, 25, a housewife, residing in Rome, wife of the declarant (both Italian).
The child was presented to the official... (and certified? by)...Dottore Giuseppe Vitigliano... on 6 June 1952, and given the name Uri, Samuele, Giuseppe.

the Scetbon's have been living in Tunisia since the XVIII century, so I doubt that they kept their Italian citizenship (they were from Livorno). If your brother's wife was Italian, than maybe he took on his Italian citizenship from her.
However, if Guido was born in Tunisia, I don't think his birth certificate will help you. It could only be of help if it was recorded in his town of origin. But, as I said above, I'm very doubtful about it.
