I would say "Chiesa Rurale della Madonna dei Martiri" (rural church of the Madonna of the Martyrs)

la chiesa navale(?) della Madonna de' Martiri (Secondigliano). Secondigliano (Napoli) was also his place of birth. I've added the question mark, as the initial letter looks like the same initial letter which is in the occupation of the first declarant.
