Jacqueline Taliaferro-Faruolo death certificate

Hello,
I am anticipating being asked for the death certificate for my father's first wife by the Italian consulate for my citizenship request.

I have a ton of information on her, but no death certificate.

Jacqueline D. Taliaferro Faruolo(married name)
Born August 24 1924 in Michigan
Died September 30 1957 in New York. She was living in Albertson, NY at the time of her death. However, it is possible that she died in a NYC hospital.

She was buried at Calvary Cemetary in Woodside, NY.

Any help on how I would be able to locate this would be greatly appreciated.

Try contacting Calvary Cemetery - Place of burial is generally recorded on the death certificate - the cemetery usually has the documentation from the funeral home. If there was no funeral, they should have the death certificate from the city.

BUT...
For an official copy - if you think she died in New York City...you should order from NYC Vital records.
