Indeed, you are right. I visited the cathedral San Panfilo and spoke to the parish priest when I was there. He did confirm that the cathedral was also referred to as Chiesa Madre by the locals.

You do NOT need to provide a death certificate. I obtained mine for a grandfather without one, so I can't see why proof of deceased would be necessary for a great-grandfather.

You just need to provide sufficient factual information to enable the searchers to locate the record!

The "atto di nascita" (birth act) can be requested either of two ways, either as an "estratto" (extract) or as a "copia integrale" (a full copy). They are both authenticated with the official stamp of the "Stato Civile" of the comune.

The extract is sufficient for the purposes of dual citizenship ...

Luke, that would have been my guess too, that, being listed first, LA VELLA was the husband's surname, while BALASSONE was the wife's. The only thing that had caused me to doubt this was the positionig of the photos above the names, which showed the wife's photo to the left of her husbands.
