Since your father is your Italian ancestor which you are applying through, your mother does not need to complete this form.

Prego, AM. I should add the dates in my translations were written in full, European format: August 30, 2011 would be written as 30 agosto 2011.

Here's a copy of a PA marriage certificate translated, from a few years ago. I removed all personal info. I don't know if PA still issues marriage certificates like this, but it was in paragraph form with info from the ledger, rather than an official-looking form like all other certificates I had. H...

Thanks Carmine for your explanation. Like the OP, I have 2 ancestors who went from Vincenzo to James, and I have always wondered why they did not go by Vincent.
