This is a copy of the Liste di Leva (Conscript List) which gives Anselmo Porta's name, his parents’ names, place of birth, date of birth, etc which you probably already know. I’m understanding that he didn’t pass the medical/physical test due to “chest tightness” (sistrettenza di torace).

Regarding his death, it says that he died in the town of Molfetta as per the Death Act of the same town, Act No 62 lì(?) - 28 January 1925.
This is what I am reading in the previous notes:
Soldato di Leva 1st Categoria Classe 1898
