Translating an Italian Birth Act

Hello,
This birth record is for the baby brother of my grandfather, Pasquale Mazzoli. There were twins, Settimia and Octavio. They both died, as did their mother (my great grandmother, Santa Filomena Mazzoli), in March 1895 (?).

Thank you PippoM and Tessa78. I believe that the photo postcard is from a maternal relative in Narni, Italy (Umbria).
But, if it is from the paternal side of the family, we do have relatives in the Abruzzo region.
